Please print and complete any of the forms below (as requested) and return them to Mrs. Sanchez in the SFDS Health Office

• Emergency Card *required (one for each family)
• Health Office Protocols (Please Review) 
• Medication Authorization Form (if applicable)
• Inhaler Permission Form  (if applicable)
• Walk Home Permission Form  (if applicable)

Additional 'Beginning of the School Year' Forms

• SFDS Parent and Student Handbook (Please Review) 
• Acceptance of School Handbook Form *required (one for each family)
• Permission to Publish Form *required (one for each new student, including incoming siblings.) Familes may also complete a new form if they’d like to update the permissions we have on file.
• Technology Agreement *required (one for each student)
